15-Minute Consult Call: Free
50-Minute Individual Session: $160
80-Minute Extended Session: $240
While I do not accept insurance directly, I can provide you with a “superbill” upon request that you can submit to your provider for possible reimbursement.
You may want to contact your insurer to address the following questions:
1) Do you have out-of-network mental health benefits?
2) Does your policy cover 50-minute sessions (CPT code 90834)?
3) If so, what is the reimbursement percentage, and does this percentage apply to a maximum amount?
4) Are there any deductibles or co-pays for which you are responsible?
5) What is the process for submitting claims? What information do they need?
